Special OUT OF TOWN entry form for Regular Players

Pick 'em Rules are as follows:

1. $1 entry fee - Fee must be paid when entry sheet is turned in and stapled to White copy!

2. Select your winners for all games this week. Write out the City or State name (whichever applies)

3. Rank your winners from 15 to 1 (for 15 game week)

4. If there are not 15 games, write your top pick on the 15 point line and work down.

5. Pick 'em sheet must be turned in BEFORE kickoff of the first game of the week, including Thursday games - NO EXCEPTIONS!

6. One entry per player. If more than one entry is submitted, all entries are void for that player. Handwriting IS checked!

7. If there is a tie for weekly or monthly score, the commish will draw the names from a hat.

8. Entry MUST be initialed by a game official or the commish.

9. If any team or game is entered more than once per entry,  neither line will be valid and neither will count

10. A bonus can be earned ONCE a week.  Bonus rules and value are posted on Bonus sign-up sheet.

The winner will receive the entry fees for all players that week less $1. The $1 will go back to the LPOTTP. (If your game was that bad you should get your money back) Winning value will be determined by the number of players that are entered each week.  There are also cash winnings for the high score each month and for the high score of the year.  It is in your interest to play EACH week.

Keep your yellow copy until after the scores are posted.   This will protect the player in the event that there has been an error in grading, or if the white copy has been lost. this is WHY the entry MUST be initialed by a game official or the commish.  The scores are posted around Wednesday or Thursday each week.
